Sutton Animal Hospital is happy to welcome new clients. Please call us at 212-935-0823 to schedule an appointment for your pet. Please click on the New Client Form blue button, download the form and complete it prior to your appointment.
Please call us if you have any questions while completing it. If possible please e-mail or fax your pet’s previous medical record and the completed new client form to us prior to your appointment.

Payment is due at the time services are rendered. Accounts must be paid in full before your pet is discharged.
